Admission begins with a connection to the facility case manager and completion of an application. A referral is requested as part of the application process.

No. Hope Transitions is a transitional housing and life skills development program, not an emergency shelter or behavioral health treatment facility. When appropriate, staff can help applicants identify a more suitable level of care.
